Assessing Risk is now part of working with young people, this guideline explains what they are about, who is responsible for doing them and then takes you through how you should do it, step by step.
There is an Ecclesiastical Insurance template form that you can use if it helps. The aim is to provide something that is suitable to the activity you are undertaking; it doesn't have to be perfect but it doea need to show that you have thought through the issues and taken any mitigating action if necessary. On a simple level it is writing down what you do naturally. For example if you are taking the children's groups to the park, crossing a road along the way you will think about the team you have, where they should be, how many you need, etc in order to get the group there and back safely with a fun time in between. That is the essence of your risk assesment - you need to write it down.
