Consent Forms - A DYS Guideline
Details
In a number of guidelines we have referred to consent forms. It is important that consent is obtained for involving children and young people in the activities being organised.
We recommend annual registration and consent form for the normal activities of the programme/group. However, there will always be additional activities that require a specific consent form, even medical consent if taking them away for a day or residential.
The following forms are available here as both pdf and word documents, which will enable you to ‘top and tail' them for your own group/church.
